Summary of Procedures for Oil, Gas, and Metallic Minerals Operations
A. PROCEDURES TO APPLY FOR A PERMIT TO DRILL
OR DEEPEN A WELL FOR OIL, GAS, OR METALLIC MINERALS
1. The following three forms must be filed with and
approved by the State Geologist before a well can be spudded.
a. Form GSB-1 - Application to Drill or Deepen a Well for Oil, Gas, Metallic
Minerals, Geological Information, Storage of Dry Natural Gas or Liquid Petroleum Gas.
This form must be filled out in detail and signed by the lessee or operator or duly
authorized representative. A plan drawn to scale by a competent surveyor or engineer,
showing the exact location of the proposed well, must accompany the application.
b. Form GSB-2 - Organization Report. This report must be filed with the first
application submitted by an applicant. Additional reports are not necessary upon making
subsequent application(s) unless a change has been made in the organization.
c. Form GSB-3 - Bond for Conformance with Laws, Rules and Regulations Governing Oil,
Gas, and Metallic Mineral Operations in the State of Iowa. Bond must be by an
acceptable corporate surety authorized to do business in Iowa.
2. A required $50.00 fee must accompany each application. Make checks or money orders
payable to the Iowa Department of Natural Resources. No permit will be granted until all
of the above requirements have been met.
B. PROCEDURE TO BE FOLLOWED UPON COMPLETION OF A WELL
1. Upon Completion of a Producer the Following Forms
Must Be Filed:
a. Form GSB-6 - Well Completion or Recompletion Report and Well Log. This form
shall be filed not later than ten days after well is completed. The well log shall be
filed not later than ten days after drilling is completed and may be recorded on the
reverse side of Form GSB-6 or submitted separately.
b. Form GSB-9 - Producers Certificate of Compliance and Authorization to Transport
Oil or Gas From Lease. This certification shall be executed and filed covering
each lease from which oil and gas is capable of being produced. No movement of oil or gas
from a lease is authorized until this form has been properly executed and approved by the
State Geologist.
c. Any electrical or other geophysical logging or surveying of the well shall be
recorded and one copy furnished to the State Geologist.
2. Upon Completion of a Dry Hole the Following Forms Must Be Filed:
a. Well Log. The well log shall be filed not later
than ten days after drilling is completed and may be recorded on the reverse side of Form
GSB-7 or submitted separately.
b. Form GSB-5 - Sundry Notices and Reports on Wells. Notice of the proposed
method of abandoning and plugging any well drilled in connection with oil, gas, and
metallic minerals operations must be filed on Form GSB-7, and approval obtained from the
Office of the State Geologist prior to commencing operations. Time must be allowed for his
representative to be present at the plugging operations, if so desired by the State
Geologist. Where time required to file notice and obtain approval in writing would
constitute an undue hardship, verbal permission to proceed may be granted, but in any case
the form must be filed.
c. Form GSB-7 - Well Plugging Record. Within ten days after the plugging of any
well has been accomplished, the operator thereof shall file said Form GSB-7, with the
State Geologist, setting forth in detail the method used in plugging of the well.
d. Any electrical or other geophysical logging or surveying of the well shall be
recorded and one copy furnished to the State Geologist.
NOTE: General rules regarding drilling etc., have been promulgated by the Iowa
Department of Natural Resource and will be furnished upon request. Instructions regarding
use of each form are printed on the reverse thereof. All forms are to be filed in
triplicate, unless the instructions specifically provide otherwise. If approved copies
are desired for applicant's files, additional copies must be submitted.
Prompt submission of properly completed forms and strict compliance with the laws,
rules, and regulations governing oil, gas, and metallic minerals operations in Iowa will
avoid much time-consuming correspondence and unnecessary delay.
